for 12 months (which I did for 2) you will receive $25 back. For 6 months
you will receive $10.
To receive the form on line was a pain, did not like my email etc. so I called.
The company will mail you the form, You must cut off the black box for proof of purchase with the form they send you its left of LOT EXP.
I purchased the meds from the vet, so they would have data regarding the med my two shelties receive.
