Having gone through a dog with a blockage and catching it just in kind, I would see vet advice, as Chris has suggested.
our sheltie has not eaten in three days now!

He vomited the tuna..which he loves
Heck a few months ago, a friend gave him a can and he wolfed it down..without my knowledge. He loves cheese, and rejects it.
He has gone from 36 bls down to 27 bls now. The vets here in Delta BC south of Vancouver, which are even more expensive, are pricy. Asked my wife, how much do they charge just to have him stay in a cage over night and she said $150.00 which includes observation. I said WOW, that's alot!
What are the typical exams for my dogs condition? What should I ask and will the vet tell me up front, the cost ? I may have to take him to Bellingham in Washington, where it could cost less to have him looked at.

Vomiting, inability to keep food down, diarrhea, lethargy ... everything you've described so far. It's life threatening and really needs to be addressed immediately.
Our Husky used to swallow socks whole, which caused a very high risk of blockage. Our vet suggested giving him a tablespoon of mineral oil to lubricate the passage of a bulky object. It did help.
But, and I can't stress this enough, given the description you've provided with the time passed and weight loss, it's imperative that he be treated medically, both for the malnutrition and the cause, if it is a blockage.
Has he been x-rayed?
Vomiting or not eating ? Lethargy, diarrhea or less bms...does he usually eat stuff ? If a foreign body is suspected the vet will want to do a barium series/x rays and will give you an estimate of cost if you ask...Is he still on antibiotics ? they can cause GI upset too. We are just guessing- your vet can actually help him so you really should have him seen. Praying he is better.

Have all the previous consults been with the same vet? If you can afford to take him, it sounds like it's time for a second opinion, maybe with a specialist. Trying a vet college sounds like a good idea. I'd also explain you're tight on cash, as they should give you some flexibility in the tests they run and whether they keep him in for observation.

We have been using the same vet for the past few years and is one of the most experienced and knowledgeable vets in the lower mainland. She also cares for the Delta Police K9 unit and also, assistant to Caesar Milan the dog whisperer when he was in town to do his show on stage.
The bill for all the exams are as follows:

Sad news, We had to put our sheltie down today 
The lab results came back about three weeks ago. He had appendicitis and failing kidneys. Plus the fact he had ecoli three years running now, we had spent 425.00 two weeks ago, to take him in because he was becoming lethargic and not eating. Went from 36 bls to 27 bls. He perked up with the IV, but it was only temporary and did eat for a while, then stopped eating again for three days. Most everything given to him rejected. We could not afford another $425.00 night stay with IV so yesterday, got the news from the wife we were to put him down this morning. At first, I did not want to go because it would depress me but eventually, I did. I was in the exam room with my wife while the veterinarian administered the lethal dose of surum. Very sad to see him die before our eyes but it was quick and painless. We will have him cremated and put into a white earn with a paw print and name next to it.

For all those owners who own a Sheltie, please PLEASE be very observant where your dog puts his nose at. He could end up eating something off the ground that has ecoli in it and cost you thousands of dollars to treat it, just like us.
